Taiwan news reports that a Japanese journalist heard from a Chinese doctor treating the North Korean leader that he is in a coma.

The reality is if he's already brain dead or on life support likely to die no clarification will ever be given until the Kim family have secured the power base required to keep them in charge.
Things could get very interesting very quickly if that was to be challenged by a senior military figure but who even knows if that would produce a better outcome for the North Korean people.

Kim Jong-chul (older brother) - Middle child of Kim Jong-il. Apparently not involved in politics. Said to be a big Eric Clapton fan and loves playing guitar. His father described him as being "too girly" to lead North Korea.
Kim Yo-jong (younger sister) - Plays an active role in the regime, and has risen through the ranks in the last few years.
Unnamed son of Kim Jong-Un - Oldest son is said to be about 10 years old, but obviously too young to be leader at this stage. He could be a leader in waiting once he reaches adult age, and one of the top ranking officials within the Workers Party is temporary leader until then.
Choe Ryong Hae - Would breakaway from the Kim Dynasty, but he's the military second-in-command behind Kim. Kim's younger sister is reported to be married to his second son.
Kim Ba Singer - maybe past her prime but still worth a shot
Kim Ka Dashian - keen to get the big guns out.
